New patients may request an appointment by clicking the Client Portal. All Intake Forms are in the Client Portal. All new patients must join the Client Portal and sign and complete all forms as soon as possible.
Patients may be referred to Synergy Counseling Center by a friend, physician, specialist, hospital, pastor, institution, etc. Please complete a Referral Form by clicking below.
